Rarer superficial cases present on the skin of the hands and feet as smaller tumors, which generally carry a more favorable <span data-term-id=\"241\">prognosis<\/span>.<\/p>\n<h2>Histological Characteristics of Synovial Sarcoma<\/h2>\n<p>The <span data-term-id=\"128\">histology<\/span> of synovial sarcoma can present in three main forms: biphasic (comprising spindle and <span data-term-id=\"1667\">epithelioid<\/span> cells), monophasic (containing only spindle cells), or undifferentiated (which often require <span data-term-id=\"1488\">cytogenetic<\/span> testing for definitive recognition). These malignancies are usually found deep within the <span data-term-id=\"1184\">soft tissues<\/span>, although they occasionally appear in the <span data-term-id=\"61\">dermis<\/span> (Figure 1). The <span data-term-id=\"1660\">spindle cells<\/span> are typically arranged in plump <span data-term-id=\"1674\">fascicles<\/span>, exhibiting distinct hyalinization and lobulation (Figures 2, 3). Una <span class=\"term\" data-term-id=\"1849\">translocaci\u00f3n<\/span> caracter\u00edstica (t(X;18)(p11.2;q11), que involucra los <span class=\"term\" data-term-id=\"775\">genes<\/span> SYT-SSX1 o SYT-SSX2) se identifica en el 90% de los casos.<\/p>\n<h2>Diagn\u00f3stico Diferencial del Sarcoma Sinovial<\/h2>\n<p>El <span class=\"term\" data-term-id=\"813\">Diagn\u00f3stico diferencial<\/span> debe considerar otros <span class=\"term\" data-term-id=\"1666\">sarcomas<\/span>; las variantes monof\u00e1sicas del sarcoma sinovial pueden simular varias otras entidades malignas. Por ello, casi siempre se requiere una bater\u00eda completa de estudios inmunohistoqu\u00edmicos y citogen\u00e9ticos.<\/p>\n<p>Respecto al adenocarcinoma <span class=\"term\" data-term-id=\"1000\">Metast\u00e1sico<\/span>: la <span class=\"term\" data-term-id=\"187\">morfolog\u00eda<\/span> presentada en un sarcoma bif\u00e1sico con \u00e1reas epitelioides prominentes puede asemejarse a un adenocarcinoma. La presencia de componentes sarcomatoides y el an\u00e1lisis de estudios complementarios son cruciales para diferenciar estas patolog\u00edas.<\/p>\n<p>En cuanto al <span class=\"term\" data-term-id=\"2157\">tumor<\/span> maligno de la vaina <span class=\"term\" data-term-id=\"1374\">perif\u00e9rico<\/span> del <span class=\"term\" data-term-id=\"176\">nervio<\/span> (MPNST): la exclusi\u00f3n de este diagn\u00f3stico puede ser particularmente compleja, dado que algunos sarcomas sinoviales expresan S100 (un marcador tambi\u00e9n presente en el MPNST).
