Cl\u00ednicamente, la <span class=\"term\" data-term-id=\"164\">lesi\u00f3n<\/span> inicial t\u00edpicamente se manifiesta como una <span class=\"term\" data-term-id=\"235\">placa<\/span> de color cut\u00e1neo, que evoluciona hacia una apariencia m\u00e1s rojiza\/marr\u00f3n y desarrolla una superficie <span class=\"term\" data-term-id=\"733\">nodular<\/span>.<\/p>\n<h2>Histolog\u00eda del Dermatofibrosarcoma Protuberans<\/h2>\n<p>Histopatol\u00f3gicamente, el dermatofibrosarcoma protuberans se caracteriza por una <span class=\"term\" data-term-id=\"893\">proliferaci\u00f3n<\/span> de c\u00e9lulas fusiformes localizadas profundamente en la <span class=\"term\" data-term-id=\"61\">dermis<\/span>. Con la progresi\u00f3n, estas <span class=\"term\" data-term-id=\"2115\">lesiones<\/span> tienden a infiltrar el tejido graso <span class=\"term\" data-term-id=\"900\">subcut\u00e1neo<\/span> (ver Figuras 1 y 2). Es posible observar invasi\u00f3n muscular. Si bien frecuentemente se respeta la capa superficial, la afectaci\u00f3n de la <span class=\"term\" data-term-id=\"85\">epidermis<\/span> junto con la <span class=\"term\" data-term-id=\"1278\">ulceraci\u00f3n<\/span> son eventos poco comunes pero posibles. Las c\u00e9lulas tumorales fusiformes exhiben una apariencia relativamente uniforme, caracterizada por <span class=\"term\" data-term-id=\"729\">n\u00facleos<\/span> alargados y un m\u00ednimo o nulo <span class=\"term\" data-term-id=\"1539\">pleomorfismo<\/span> (Figuras 3 a 5). Se observa una positividad caracter\u00edstica para la prote\u00edna <strong>CD34<\/strong> (Figura 7). Es fundamental notar que este tumor es <strong>CD31 negativo<\/strong> y tambi\u00e9n resulta negativo para el <strong>actina de m\u00fasculo liso<\/strong>.<\/p>\n<h2>Diagn\u00f3stico Diferencial: Separando el DFSP de Otras Lesiones Similares<\/h2>\n<p>Establecer el diagn\u00f3stico diferencial preciso para el Dermatofibrosarcoma Protuberans es crucial, ya que varias lesiones pueden presentar caracter\u00edsticas histol\u00f3gicas superpuestas. A continuaci\u00f3n, se detallan las entidades a considerar:<\/p>\n<h3>Dermatofibroma Celular<\/h3>\n<p>Esta entidad se caracteriza por una proliferaci\u00f3n radial de c\u00e9lulas que exhiben n\u00facleos de mayor tama\u00f1o. Puede ser evidente la presencia de dep\u00f3sitos de <strong>hemosiderina<\/strong>. A diferencia del DFSP, la porci\u00f3n central de un dermatofibroma celular es <strong>negativa para CD34<\/strong>, pero resultar\u00e1 positiva para el Factor XIIIa.<\/p>\n<h3>Sarcoma D\u00e9rmico<\/h3>\n<p>El <strong>Sarcoma D\u00e9rmico<\/strong> se distingue del DFSP por la presencia marcada de <strong>polimorfismo nuclear<\/strong> y una frecuencia elevada de <strong>mitosis<\/strong>, caracter\u00edsticas que lo diferencian claramente del curso biol\u00f3gico del dermatofibrosarcoma protuberans.<\/p>\n<h3>Leiomiosarcoma<\/h3>\n<p>El <strong>Leiomiosarcoma<\/strong> es un tumor compuesto por <strong>c\u00e9lulas fusiformes pleom\u00f3rficas<\/strong>, con n\u00facleos que frecuentemente adoptan una forma de cigarro, junto con un notable aumento en la tasa de mitosis. Una caracter\u00edstica distintiva es su positividad para el <strong>actina de m\u00fasculo liso<\/strong>.<\/p>\n<h3>Melanoma Desmopl\u00e1sico<\/h3>\n<p>Para distinguir el <strong>Melanoma Desmopl\u00e1sico<\/strong>, se deben buscar caracter\u00edsticas adicionales como la presencia de <strong>fibrosis<\/strong> extensa y <strong>linfocitico infiltrados<\/strong> de tipo <strong>focal<\/strong>. Las c\u00e9lulas tumorales en este caso son t\u00edpicamente positivas para el marcador <strong>S100<\/strong>.<\/p>\n<h3>Neurofibroma<\/h3>\n<p>El <strong>Neurofibroma<\/strong>, otro tumor de c\u00e9lulas fusiformes, tambi\u00e9n resulta positivo para la tinci\u00f3n S100. Adem\u00e1s, es com\u00fan encontrar la presencia de <strong>Mastocitos<\/strong> en su matriz.<\/p>\n<h3>Dermatofibrosarcoma Protuberans Fibrosarcomatoso<\/h3>\n<p>Esta variante representa una fase de progresi\u00f3n del DFSP. Se manifiesta con un patr\u00f3n focal fascicular, a veces descrito como en \"espina de pescado\". Cl\u00ednicamente, el tumor se vuelve considerablemente m\u00e1s celular que el DFSP cl\u00e1sico, exhibiendo un mayor pleomorfismo nuclear y un incremento en el <strong>recuento mit\u00f3tico<\/strong>.
