This delay is usually due to delays in performing the <span class=\"term\" data-term-id=\"24\">biopsy<\/span> and subsequent histopathological confirmation. Risk factors predisposing to this condition are, to a large extent, parallel to those observed in adult melanoma.<\/p>\n<h2>Histological Classification and Variants of Pediatric Melanoma<\/h2>\n<p>Pediatric melanoma can arise from a large congenital melanocytic nevus, <span class=\"term\" data-term-id=\"1036\">congenital<\/span> or manifest as a Spitzoid melanoma, <span class=\"term\" data-term-id=\"733\">nodular<\/span>, or superficial spreading type. The precise determination of the histology <span class=\"term\" data-term-id=\"128\">histology<\/span> is the fundamental step for adequate clinical classification.<\/p>\n<p>The clinical case examined below documents a nodular melanoma found on the arm of an 8-year-old patient (illustrated in Figures 1 through 5). This is where molecular techniques such as fluorescence in situ hybridization (<span class=\"term\" data-term-id=\"1237\">FISH<\/span>) prove valuable for increasing diagnostic reliability. In the particular case illustrated, FISH analysis showed a notable prevalence of genetic alterations.<\/p>\n<\/section>\n<p>of <span class=\"term\" data-term-id=\"1935\">abnormalities<\/span> of chromosomal abnormalities, which is consistent with the aggressive nature of the melanoma.<\/p>\n<p>Furthermore, immunohistochemical analyses are crucial for determining high rates of <span class=\"term\" data-term-id=\"895\">proliferation<\/span> cell proliferation, using the Ki-67 marker, and for confirming pagetoid invasion using the Melan-A marker.<\/p>\n<section class=\"textBlock imageLinkBlockTitle\">\n<h1 class=\"colour--primary\">FISH Results in Pediatric Melanoma<\/h1>\n<\/section>\n<section class=\"imageLinkBlock\">\n<div class=\"flex\">\n<div class=\"imageLinkBlock__item__image\"><img decoding=\"async\" src=\"https:\/\/dermatly.com\/wp-content\/uploads\/2020\/11\/childhood-melanoma-fish__ProtectWyJQcm90ZWN0Il0_FocusFillWzI5NCwyMjIsIngiLDFd.jpg\" alt=\"Image showing FISH findings with chromosomal abnormalities in a case of pediatric melanoma.\" \/><\/div>\n<p class=\"p-small\">Special Stains and Syndromes Associated with Sebaceoma<\/p>\n<\/div>\n<\/section>\n<div class=\"clearfix imageLinkBlock\"><\/div>\n<p>Abnormal findings observed via FISH included a gain of the RREB1 gene (located at 6p25 and marked with a red probe) and a loss of the MYB gene (located at 6q23 and marked with a yellow probe).<\/p>\n<h2>Approach to the Differential Diagnosis of Pediatric Melanoma<\/h2>\n<p>The main challenge in the differential diagnosis <span class=\"term\" data-term-id=\"813\">mucous membranes.<\/span> of pediatric melanoma involves distinguishing it from other neoplastic entities, mainly:<\/p>\n<ul>\n<li><strong>Spitz Nevus:<\/strong> The most ambiguous cases may require consultation with a specialized center and the performance of additional <span class=\"term\" data-term-id=\"1488\">cytogenetic<\/span> analysis to reach a definitive confirmation.<\/li>\n<li><strong>Proliferative Nodules within Congenital Nevi:<\/strong> Similar to Spitz Nevus, lesions presenting doubtful characteristics require an exhaustive external evaluation and the application of detailed cytogenetic tests.<\/li>\n<\/ul>\n<p>Ensuring the precise identification of pediatric melanoma is fundamental, which is achieved through a rigorous correlation between clinical findings, advanced histopathology, and specific molecular studies, thus guaranteeing the most appropriate management for each young patient.<\/p>","protected":false},"excerpt":{"rendered":"<p>Melanoma Infantil: Comprender su Diagn\u00f3stico y Caracter\u00edsticas Patol\u00f3gicas El melanoma que afecta a la poblaci\u00f3n infantil, aunque estad\u00edsticamente poco com\u00fan, presenta desaf\u00edos significativos, ya que frecuentemente se detecta cuando la enfermedad ya se encuentra en estadios cl\u00ednicos avanzados.
